Sikar : Health Department will create referral register at PHC level. In the referral register, the information about which hospital the patient was referred, what was the disease due to which he had to be referred, this information will have to be clearly mentioned in the referral card. Along with this, the patient’s address and mobile number will also have to be entered. The Administrative Secretary of the Health Department has instructed the CMHO to make arrangements in this regard. Patients referred from PHC will be audited every month. In the audit, it will be investigated what situation arose in the PHC that the patient had to be referred. The patient arrived suffering from some disease or got injured in an accident.
Information about the presence of doctors and staff in the PHC will be taken. An investigation will be conducted regarding medicines and testing facilities. Cross verification will be done from the institution to which the patient was referred. After treatment, information about the patient’s illness will be taken from the doctors, so that it can be found out whether the patient actually needed to be referred to the PHC or not. If treatment was possible in PHC, why was it not done? A plan will be prepared after finding out the reason for referral, so that the referred patients can get treatment at the local level.

CMHO, Sikar Actually, the number of patients referred at PHC-CHC level is continuously increasing. In such a situation, the pressure of patients in the district hospital is increasing. Pregnant women are not able to get delivery facilities at PHC-CHC. They are referred at the district level. To reduce the number of referral cases, the Health Department will provide training to the doctors and staff appointed at the PHC level. They will be trained for treatment of delivery and other diseases.
